This form is a general example for setting down for yourself what happened when you were hurt at work. It is not the industrial accident claim form itself and it is not legal advice. The claim for medical care benefits is made to the Korea Workers' Compensation & Welfare Service on its own form. Check the procedure and the documents needed with the Service (1588-0075). An industrial accident claim can be filed by the worker directly, without the employer's consent.
