About this template

What this form is

The file opens with Medical Record Form MR-01, the title Clinical Note Template and the subtitle Outpatient Progress Note. It records a single outpatient visit on one page and runs through six numbered blocks: ① Patient information, ② Vital Signs, ③ Chief complaint and history, ④ Progress Note (SOAP), ⑤ Prescription (Rx), ⑥ Follow-up measurements. It does not replace an EMR – it is for writing during the consultation, for transcription later, or for teaching.

Filling order

  1. ① Patient information: Registration No., Name, Sex / Age, Date of birth and Insurance type in the upper table; Visit date & time, Department, Attending physician, Visit type (First visit / Follow-up / Emergency) and Accompanied by below. Insurance is one of four boxes: NHI, Medical Aid, Auto, Work injury. The visit date cell already carries the year 2026, so edit the master before printing in a later year.
  2. ② Vital Signs: BP (mmHg), Pulse (bpm), Resp. (/min), Temp. (℃), SpO₂ (%) and Weight (kg), with two entry rows. The note under the table says “Use the second row for values re-measured after treatment” – leave it blank if there was none.
  3. ③ Chief complaint (C.C.), Present illness (P.I.), Past history (P.Hx), Medications (Med), Allergies, and Social / Family history. Past history is a tick-only row running HTN · DM · TB · Hepatitis · Surgery · None; for Allergies, tick “Yes” and name the drug or food in the brackets. The social row holds smoking in pack-yrs, alcohol use and family history on one line.
  4. ④ SOAP: the S, O and A cells each carry a grey prompt line, a “Points to cover” label and four checklist bullets, with one blank cell underneath. The prompts and the checklist are sample text; write the actual record in the blank cell.
  5. ④ The Plan table has four columns – Type, Plan details, Scheduled date, Checked – and five pre-printed rows: Medication, Tests · procedures, Education · lifestyle advice, Follow-up, Return visit · referral, plus one blank row.
  6. ⑤ Prescription: six columns (No, Drug / procedure name, Dose, Directions, Days, Notes) over six numbered rows, with the outside-prescription and test-order boxes underneath.
  7. ⑥ Follow-up at four time points – First visit, 3 days, 1 wk, 2 wks – with a Measured value row and an Assessment row. Decide which measure you are tracking (temperature, blood pressure, pain score) before printing, or the cells mean nothing.

Examples

  • ② First row 128/82 · 88 · 18 · 37.9 · 96 · 64.2; second row, 30 minutes after an antipyretic, 124/80 · 78 · 16 · 37.1 · 98.
  • ④ S: dry cough for two days, chills since last night, pain 3/10.
  • ④ O: pharyngeal redness, clear breath sounds bilaterally. A: acute upper respiratory infection suspected, pneumonia to be excluded.
  • ④ P row: Type “Tests · procedures”, plan details chest X-ray, scheduled today.
  • ⑥ 37.9 → 37.1 → 36.8, assessment improving.

The four numbers at the foot

The Reference values block (36.8℃ for Temperature, 120/80 for Blood pressure, 72 for Pulse per min, 98% for Oxygen saturation) is an adult normal range, not this patient’s data. Do not overwrite it with measured values, or it will contradict ②. The line below it, “The patient’s condition, exactly as it is”, is a fragment rather than a sentence, so finish or delete it before printing. ③ and the Plan table look alike but are not: “Medications (Med)” in ③ is what the patient already takes, while “Medication” in the Plan is what you are prescribing now – do not copy one into the other.

Where it goes wrong

  • Filing the note with the Recorded by block (Recorded on, Attending physician (signature), Resident / Nurse) left empty – nobody can attribute the record.
  • Mixing S and O. As the O prompt says, record only what was observed or measured and leave interpretation to A; what the patient says belongs in S.
  • Leaving the checklist bullets in place and writing underneath them, so sample and record blend together. Lines such as “Recheck vital signs (BP · HR · RR · BT · SpO₂)” in O, or “Differential diagnoses and the grounds for ruling each out” in A, are prompts to check, not content to keep.
  • This form holds sensitive data. As the closing line states, copies are released only on application by the patient or a legal representative, after identity verification; never leave a part-filled sheet outside the consulting room.
