About this template

When to use it

A one-page Leave Request Form covering every kind of leave a Korean workplace runs, not annual leave alone. The header cells read Date of request / Affiliation · Name / Type of leave / Document No., sampled as 2026-08-18, “Lee Su-min, Corporate Support Team”, Annual paid leave and HR-L-2026-0231. Which type you tick drives everything else on the page, so start there.

Filling order

  1. 1. Type of leave and dates — choose from annual, half day, family event, sick or official leave, then the dates. The sample runs September 7 to September 9, 2026, 3 days. A half day is defined on the page as 09:00-13:00 in the morning or 14:00-18:00 in the afternoon.
  2. 2. Reason and supporting documents — the paperwork differs by type. Annual leave can be requested without stating a reason; family-event leave needs evidence such as a family relation certificate; sick leave needs a medical certificate or proof of treatment.
  3. 3. Cover arrangements and contact — who covers (the sample names Choi Do-hyun, Corporate Support Team, ext. 2140), a number you can be reached on, and a line about handing urgent matters to the team lead before you leave.
  4. Sign the three-cell block: Applicant / Team lead / HR Team.

Annual leave is a right, not a favor

The reason row is the part people get wrong. Annual paid leave under the Labor Standards Act is taken on the employee’s own timing, and the employer may move it only where granting it at that time would seriously disrupt the business. That is why the file says annual leave can be requested without a reason, and why the reason and evidence rules in section 2 attach to the other leave types instead. If your workplace runs its own rule requiring a reason for annual leave, that is a house rule and it sits above the statutory minimum, not inside it.

The four leave figures must agree

The Key metrics table reads 15 days, 7 days, 8 days and 3 days, labeled leave accrued, days used, days remaining and this request. Accrued minus used is what remaining has to equal — 15 minus 7 is 8 in the sample — and this request has to fit inside remaining. Anyone approving the form checks that arithmetic first, so update the ledger and the form together rather than after the fact. Annual leave accrues by length of service, so the accrued figure changes at each anniversary and should not be copied from last year’s form.

Timing and lateness

  • Annual leave: give notice ahead of time, three business days in the sample workplace, so cover can be arranged.
  • Family-event leave: report as soon as the event occurs; the paperwork can follow.
  • Same-day sick leave: call it in, then submit the written form afterward. Filing nothing at all is what turns a sick day into unexplained absence.
  • Unpaid leave: agree it before you submit. It affects payroll and, where it runs long, the four social insurances.

When you come back

Two things close the loop and both are easy to skip. Take a handover from whoever covered, so nothing sits untouched in your absence, and submit the original supporting document if you were away on family-event or sick leave — a copy sent ahead by message is not the record. Then have the leave ledger updated so the remaining balance on your next request starts from the right number.
